Referrals to Our Active Communities Programmes

Our Specialist Rehabilitation Exercise programmes support individuals recovering from illness or managing long‑term health conditions. Our programmes are designed and delivered by qualified specialists to help people improve strength, mobility, confidence and overall wellbeing.

We welcome referrals from health and social care professionals, and self-referrals into all areas of our Active Communities offer. If you work with individuals who would benefit from becoming more active, whether to improve physical health, enhance mental wellbeing, or build social connection, our team can help you identify the most suitable programme.
